§2 Consents given in the form
By completing the poultry insurance calculator form on the INSTA-CATCH website, you are asked to give the following consents:
Consent 1 — Contact regarding insurance (required)
I consent to being contacted by JB Broker Jarosław Szkop and INSTA-CATCH Dawid Siliwoniuk regarding poultry insurance, including sending an insurance offer from Compensa TU S.A. Vienna Insurance Group via the contact details I have provided (phone, e-mail).
Consent 2 — Data processing by the broker (required)
I consent to the processing of my personal data by JB Broker Jarosław Szkop (data controller) for the purpose of preparing an insurance offer, analysing insurance needs, and handling the poultry insurance process, in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (GDPR).
I declare that I have read the Insurance Privacy Policy.
Consent 3 — Acknowledgement of the brokerage power of attorney (optional)
I declare that I have read the Brokerage Power of Attorney granted to JB Broker Jarosław Szkop in the scope of insurance brokerage regarding poultry insurance.
Consent 4 — Contact from INSTA-CATCH (optional)
I consent to being contacted by INSTA-CATCH Dawid Siliwoniuk regarding the INSTA-CATCH platform and related services for poultry farmers.
Consent 5 — Farm order handling (optional)
I consent to being contacted by INSTA-CATCH Dawid Siliwoniuk regarding farm order handling and services related to poultry production.
§3 Information on rights
You have the right to access your data, rectify them, erase them, restrict processing, transfer data, and object to processing.
Consent may be withdrawn at any time, without affecting the lawfulness of processing carried out before withdrawal, by contacting: jb.broker@wp.pl.
You also have the right to lodge a complaint with the President of the Personal Data Protection Office (ul. Stawki 2, 00-193 Warsaw).
§4 Retention period
Personal data will be stored for the period necessary to achieve the purposes for which they were collected, and thereafter — for the period required by law or until consent is withdrawn.
