Privacy Policy — Poultry Insurance
This Privacy Policy sets out the rules for processing personal data in connection with insurance brokerage services for poultry insurance provided through the INSTA-CATCH service.
§1 Data Controller
The controller of personal data processed in connection with poultry insurance is:

The INSTA-CATCH service (INSTA-CATCH Dawid Siliwoniuk, Tax ID (NIP): 5372610246) acts as a data processor on behalf of the Controller for the technical operation of the contact form and transmission of data to the broker.
§2 Purpose of data processing
Personal data are processed for the following purposes:
Preparing a poultry insurance offer by Compensa Towarzystwo Ubezpieczeń S.A. Vienna Insurance Group
Contact regarding insurance terms and policy preparation
Handling the conclusion and performance of the insurance contract
Handling any claims and losses
Fulfilling legal obligations incumbent on the insurance broker
§3 Legal basis for processing
Art. 6(1)(a) GDPR — consent of the data subject given by checking the appropriate boxes in the insurance calculator form
Art. 6(1)(b) GDPR — processing is necessary to take steps at the request of the data subject prior to entering into a contract (preparing an insurance offer)
Art. 6(1)(c) GDPR — processing is necessary to comply with a legal obligation to which the controller is subject (obligations of an insurance broker under the Insurance Distribution Act)
Art. 6(1)(f) GDPR — legitimate interests of the controller (handling correspondence, pursuing claims)
§4 Scope of processed data
In connection with poultry insurance, the following data are processed:
First and last name
Phone number
E-mail address
Tax ID (NIP), if provided
Number of farm facilities
Data from the insurance calculator (flock value, selected clauses, calculated premium)
Message content
IP address and browser information (collected automatically)
§5 Data recipients
Personal data may be disclosed to the following recipients:
Compensa Towarzystwo Ubezpieczeń S.A. Vienna Insurance Group — for the purpose of preparing an offer and concluding an insurance contract
INSTA-CATCH Dawid Siliwoniuk — as a data processor (website operator)
Entities providing hosting and IT services on behalf of the Controller
§6 Data retention period
Data processed on the basis of consent — until consent is withdrawn or the purpose for which they were collected is fulfilled
Data related to a concluded insurance contract — for the duration of the contract and the limitation period for claims (up to 6 years after the contract ends)
Data processed to fulfil legal obligations — for the period required by law (including the Insurance Distribution Act)
§7 Rights of the data subject
You have the following rights:
Right of access to your personal data
Right to rectification of inaccurate data
Right to erasure of data ("right to be forgotten")
Right to restriction of processing
Right to data portability
Right to object to processing
Right to withdraw consent at any time (without affecting the lawfulness of processing carried out before withdrawal)
Right to lodge a complaint with the President of the Personal Data Protection Office
